Louisville KY, 40202, fill out an application & get a duplicate. The Health Dept
is open M-F
8am-5pm (please come by 4:30pm).
You can also do it by mail at 400 E. Gray Street, Louisville KY, 40202 (this will take much longer). The Health Dept. accepts money orders or checks for this service along with an completed application with 'Duplicate' written on top.
